Team Around The Patient Project
Background
This project focusses on using the patient as a member of a multidisciplinary team to develop health and social care plan created and owned by the patient. This can be focused on mental health, alcohol issues or pain control or even over use of prescribed medication but will likely involve the whole of that person’s life where ever help is needed as seen by the individual.
It can be linked to the Group Consultation process if this was thought to be helpful and a relevant program was running.

Principles
The Team Around the Patient Project links in the Co-Production and Personalisation principles, using the patient to create a personalised care plan for themselves. This can be done in person or by virtual online meetings.

The team could include any of the following for example; Mental Health Practitioner, Clinician, Pharmacist, Social Prescribers, Social Worker, Citizens Advice Bureau advisor
