Alcohol Project
Background
Orwell PCN and our Social Prescribers are working with local charity, Turning Point, to identify and support patients struggling with problem levels of alcohol use. We have worked hard to get this project running as soon as possible, despite the Covid-19 Pandemic.
Principles
This project brings together the personalisation and collaboration principles, working in an interdisciplinary team to build with the person a personalised care plan for the patient and where helpful build a Team Around the Patient (TAPP).

Team
Social Prescribers, Turning Point, Orwell PCN member practices, and social services or volunteering (where thought helpful to address the wider health and social concerns).

The impact of Covid 19 has sadly delayed this project’s launch but we have begun the training and hope to launch soon!
